The relationship between asthma morbidity and family interaction patterns has a long and curious history (for a review, see Renne & Creer, 1985). Early ideas about psychogenic factors in asthma were guided by a psychoanalytic framework (French & Alexander, 1941). Children with asthma were believed to have unique relationships with their mothers, with the attacks somehow related to an inhibited suppressed cry by the patient for his or her mother. A large body of research characterized mothers of children with asthma as rejecting (Miller & Baruch, 1948), or overprotective and inconsistent in their disciplinary practices (Parker, 1983). Family influences were presumed to be unidirectional: mother to child. Treatment for asthma included psychotherapy for the asthmatic patient but not for the parents (French & Alexander, 1941). Later, recommended treatment for children with chronic asthma included separation from their parents, or "parentectomy" (Peshkin, 1976; Robinson, 1972), based on Peshkin's (1930) observations that children's asthma improved when they were separated from their families. This practice continued for many years, despite growing empirical evidence that removal of the child from the family did not result in remission of asthma symptoms.
Minuchin and colleagues (1975) challenged psychoanalytic ideas about asthma treatment with their "psychosomatic family" model, which proposed that specific family interaction patterns trigger physiological processes in the child that exacerbate his or her asthma. The asthma exacerbations in turn reinforce these family patterns, thus maintaining the unhealthy family organization. Although the "psychosomatic family" model has been criticized for lack of empirical support and for failing to consider positive family influences (Coyne & Anderson, 1988; Wood, 1993), it was novel in its focus on the reciprocal influences between biological and social (familial) aspects of illness, paving the way for family interventions for children with asthma. It also highlighted the role of family dynamics-not maternal characteristics--in symptom maintenance. At the same time, transactional developmental models (Sameroff & Chandler, 1975) provided another perspective on how the child and family mutually create, over time, a context for disease expression (Fiese & Sameroff, 1989). With the advent of social ecological perspectives (Bronfenbrenner, 1977), family systems views on children with disabilities and chronic illnesses broadened to include the multiple nested contexts influencing the child's development (Kazak, 1986), including the family's social support networks and the family's relationship with …
